项目摘要

本研究の目的は、アクションリサーチの手法を用いて、在宅医療を受ける子どもが成人として自立する過程を支援する実践を評価することである。医療の発展により、小児期に発症した慢性疾患患児・者が社会生活を送る期間が延伸した。子どもは身体的にも精神的にも成長し発達し続ける。成人になって以降も、身体的・精神的に緩やかな変化を遂げるが、その過程で生じる健康課題も変わっていく。そのため、小児期・思春期・青年期・成人期・高齢期と各年齢や個人の発達段階に見合った診察を定期的に受けて健康管理を行うことは重要である。しかし、医療体制や医療資源は都道府県など地域により異なり、小児期発症の慢性疾患患児・者の健康管理と医療体制のあり方の整備は喫緊の課題となっている。本年度は研究に着手するまでの準備期間とし、患者側と病院側からの視点で調査を計画した。具体的には、A病院の移行期医療連携チームのコアメンバーとして参加し、病院内で移行期医療支援を推進するための準備を行なった（活動目的は医療の連携と移行期の支援である）。定期的に開催された会議において、小児期に発症した慢性疾患患児・者が、小児科診療から成人科診療へ移行する方法を話し合いメンバー間で共有した。課題が生じた場合は対応策を話し合った。患者側の考えや意見を聞けるようインタビュー調査を計画し、研究倫理審査委員会への申請を行った。次年度は、小児科の各専門科から成人診療科の各科へ移行する患児・者と家族の診療場面に同席し、その過程を観察し移行過程を調査する。さらに、すでに診療科を小児科から成人科へ移った患者への面接調査を予定している。
