前年度に引き続き、J-PCI registryのビッグデータを用いて、若年性心筋梗塞患者の発症時の心肺停止をきたすリスクについて解析を行った。213,297人の心筋梗塞にPCIを施行した患者を対象として、背景因子や病変の特徴、発症時の症状などを検討した。50歳以下の若年性心筋梗塞患者は全体の11.2%に認められ、高齢者と比較して冠動脈リスク因子を有する頻度が少なく、冠動脈病変も一枝病変など重症度が低いものが多かったが、発症時に心肺停止を合併することが多かった。また心肺停止をきたすとその後の院内死亡率が上昇することがわかった。これらの結果から、若年性心筋梗塞の予防には、心筋梗塞を発症してからの二次予防では遅く、発症前に防ぐ一次予防が重要であることがわかった。一方で若年者は医療機関に関わることが少ないために、介入することが非常に困難であることが今後の課題である。特定健診などの機会を有効活用し、ハイリスクの受診者に対して、食生活の改善などの啓発をしていくことを今後検討していきたい。
