项目摘要

本研究の目的は、訪問リハビリテーション（以下、訪問リハ）従事者が、利用者の病状変化や急変の兆候などに気づき、自らがその状況に応じた適切な対応をとることができるようになるためのシミュレーション教育プログラム（以下、プログラム）を構築すること、また、そのプログラムの有効性を検証することである。令和2年度は、本研究の根幹となる病状変化シナリオおよび講義・実技、シミュレーションの実施案を共同研究者とともに作成した。令和3年度は、完成したシナリオをもとにシミュレーション教育プログラムを作成し、研修計画を立案するとともに、倫理審査の承認を得た。その後、令和4年2月に研修会を予定していたが、新型コロナウイルスの感染拡大に伴い、令和4年6月以降に実施を延期した。令和4年度は、延期となった研修会を実施し、教育効果の検証を行った。令和4年4月より参加者の募集を開始したが、新型コロナウイルスの影響により応募者が少なく、最終的には16名（定員40名）の応募となった。集まったリハ従事者を座学のみ（SM-群：8名）と座学+シミュレーション（SM+群：8名）の2群に無作為に割り振り、それぞれに研修を実施した。その際、筆記テスト、自己効力感評価、研修効果に関する質問紙を研修前後で評価測定し、その結果（短期効果）を2群で比較検討した。研修は無事に終了し、研修効果の評価指標を2群で分析した結果、筆記テストおよび自己効力感の得点は2群ともに向上していた。また、SM+群がSM-群と比較して聴診や触診による異常所見などのイメージが有意に向上していた。現在、研修後の効果（長期効果）について追跡調査を行っている最中であり、今後はこれらの追跡調査の結果も踏まえ本プログラムの有効性について検討する予定である。
