项目摘要

本研究は，看護職が嚥下障害を正確に臨床判断するための診断指標を明らかにするため，嚥下障害の診断指標の内容妥当性検証を行うものである．2022年度は，2021年度に実施したデルファイ調査の結果の分析及び学術学会での発表を行った．2021年度に協力の得られた摂食・嚥下障害看護認定看護師（認定看護師）275人を対象に3Roundで構成されるデルファイ調査を行った．デルファイ調査の分析は、3Roundの結果をもとに分析した．3Roundでコンセンサス基準に至らなかった診断指標は， Fehring（1987）のDiagnostic Content Validation model（DCVモデル）の分析から除外した．診断指標はDCVモデルに基づき，各回答者の評価結果を点数に換算し，各診断指標の平均値（DCV値）を算出した．DCVモデルの基準により，「major指標」「minor指標」「除外する指標」に分類し，診断指標リストの内容妥当性を分析した．分析の結果，嚥下障害を表す程度に対する回答のコンセンサス基準を満たした項目は84項目中77項目であった．この77項目は、DCV値により，major指標（17項目），minor指標（46項目），除外する指標（14項目）に分類された．これにより，嚥下障害を表す診断指標63項目を明らかにすることができた，本研究の結果は，認定看護師の意見のコンセンサスが得られた結果によるものであり，内容妥当性がある診断指標と考える．上記の研究結果は，第42回日本看護科学学会学術集会で口演発表をおこなった．

This study aims to clarify the diagnostic indicators for nursing staff to correctly judge the symptoms of respiratory disorders, and to evaluate the appropriateness of the diagnostic indicators for respiratory disorders. Analysis of the results of the survey conducted in 2021 and the development of a list of academic societies. The survey was conducted in 2021 by 275 people in response to a survey conducted in 3Round. The analysis of the results of the 3Round survey was carried out with the exception of the analysis of Fehring's Diagnostic Content Validation model (DCV)(1987). The diagnostic index is based on DCV, and the evaluation results of each respondent are converted into points. The average value of each diagnostic index (DCV) is calculated. The DCV is based on the classification of "major index","minor index" and "except index". The appropriateness of the diagnostic index is analyzed. The analysis results show that 77 items out of 84 items are listed in the table below. 77 items, DCV values, major indicators (17 items), minor indicators (46 items), except indicators (14 items) 63 items of diagnostic indicators were identified and the results of this study were satisfactory. The results of the above study were reported in the 42nd Academic Conference of the Japanese Society of Nursing Sciences.
