腰痛・膝痛などの慢性運動器痛に対する公衆衛生学的観点からのセルフマネジメント強化が重要と考えられる。近年、認知行動療法による不安や恐怖感など痛み認知の改善が注目されており、認知行動療法による慢性運動器痛改善の機序に、セロトニン（5-hydroxytryptamine：5-HT）やβ-エンドルフィン（β-endorphin：β-END）などの神経修飾因子の関与が示唆されている。本研究は、地域中高年住民の慢性運動器痛保有者に対する運動療法と認知行動療法を組合わせたセルフマネジメント支援が、生活機能を改善し、その改善に神経修飾因子が関与するかを、客観的生体指標を加えた無作為化比較試験によって検証する。運動療法や認知行動療法を用いたセルフマネジメント強化と神経修飾因子との関連解明は、地域における客観的根拠に基づく慢性運動器痛の予防の推進に貢献すると考えられる。令和2年度までに5-HTとβ-ENDの測定精度と再現性を確認した。令和3年度から令和4年度にかけては、本研究課題の検証を進めるため、国内の新型コロナウイルスの感染対策を十分に考慮し実施計画の再調整を行い、症例対照研究での仮設検証に切り替えることができた。現在、予定していた測定キットの輸出入ができなくなり、再度妥当性のある測定方法の確立に時間を要しているが、再度妥当性が確認できた測定方法で分析を進める見込みが立っている。令和5年度は、国内の新型コロナウイルスの感染状況を十分に考慮し再調整を行った実施計画に従って、本研究課題の検証を進め、結果をまとめる予定である。
