项目摘要

今年度も新型コロナウィルス感染症の流行に伴って，旅費を使用する形での史料調査を行うことができなかった。そうしたなか，代替措置として都市社会資本に関わる史料調査を可能な範囲で実施し，分析結果の一部を公表した。そのねらいは次のようなものである。本課研究課題が直接の対象とする医療サービス商品は，医師の個人的な事業も含めて社会資本としての医療機関によって生産され，直接的には傷病患者との間で取引される。この取引の背景としては，医療需要主体としての傷病患者の予備軍である住民と医療供給主体としての医療機関が，ある一定の地理的範囲内に集住していることが前提されている。その際，「ある一定の地理的範囲内」の住民が先行して増加した場合，傷病患者発生率が一定であったとしても医療需要がその供給を上回る事態が生じる。医療機関にとどまらない社会資本一般の供給が，急激に都市化が進む事態のなかで需要の増加とどのようにバランスするのかということは，本研究の大前提を確認するための重要な作業として位置付けることができる。今年度はそうした重要な前提を確認するための基礎的な作業をもって代替的な研究を実施した。具体的には，戦後において急速な宅地化と住民の急増をみた東京郊外のうち，神奈川県川崎市および横浜市の北部農村地域が大手鉄道資本の主導する開発によって宅地化され住民が急増する過程で，社会資本整備がどのように構想され実際にはどの程度需要と供給がバランスする形で増加していったのか，という問題を検討した。結果としてこの地域では，社会資本整備が後追い的に整備されるという典型的な都市問題が発生しており，とりわけ都市化のプロセスのなかでは社会資本の需要超過状態を想定しなければならない，ということが明らかとなった。この知見は，とりわけ都市社会政策および都市社会事業という本研究の課題領域にとって重要な示唆をもたらす成果である。
