本研究では、アセトアミノフェン誘発性肝障害の重症化を肝アダプテーションの能力が規定するという仮説のもと、実臨床おける多様な背景をもつアセトアミノフェン使用患者集団における肝アダプテーションの発生実態とそれを規定する因子を、電子カルテ由来の実臨床データを用いて解析し、明らかにすることを目的に、以下を実施した。1. 研究用データソースの利用申請：本研究立案時は自機関の医療情報の利用を検討していたが、より大規模でかつ多様な背景をもつアセトアミノフェン使用患者集団を包含し、かつ本研究に必要となる電子カルテ由来の経時的な検査情報を含む医療情報データベースである一般社団法人 健康・医療・教育情報評価推進機構（HCEI）が構築したRWDデータベース（全国229医療機関、約2560万人の電子カルテ由来の診療情報データを保有、2022年9月現在）を本研究のデータソースとしたいと考えた。HCEIは、データベースの利用者を公募しており、HCEIが定める条件を満たす研究に対し、学術用データセットを提供している。そこで、HCEIより本研究の学術用データセットの提供を受けるべく、利用申請手続きとして1）自機関の倫理審査委員会への申請、2）HCEIへの利用申請、3) HCEIが設置したデータ利活用審査委員会への審査申請を行った。2. データソースの抽出・研究用一次データセットの構築：臨床専門家、薬物性肝障害の有識者、および生物統計家と共同し、データ抽出条件、研究デザインダイアグラム、研究用一次データセットおよび二次データセット（解析用データセット）の設定条件を検討した。1の手続きの後、HCEIに対象症例のデータ抽出を依頼、HCEIより提供を受けたデータを用い、研究用一次データセットの構築を開始した。

This study aims to determine the ability of patients to develop severe liver disease using a patient set and to determine the factors that determine the severity of liver disease, the origin of liver disease, and the purpose of clinical diagnosis. Application for the use of research software: This study was initiated to examine the use of medical information from institutions on a large scale and in diverse contexts, including patient aggregation. HCEI offers research and academic services to users of the website. 1) Application from the Ethics Review Committee of the Agency, 2) Application for the Use of HCEI, 3) Application for the Review of the Use of HCEI 2. Construction of primary data set for extraction and research: clinical experts, knowledge of biological disorders, common knowledge of biostatisticians, data extraction conditions, research data set, setting conditions for primary data set for research and secondary data set for analysis. After the first step, HCEI will extract the data from the case, HCEI will provide the data for the first time, and the construction of the data for the first time will begin.
