【研究概要】クローン病において生物学的製剤の効果はめざましいが、薬剤費は高額である。本研究はクローン病における生物学的製剤の医療費・費用対効果について従来治療との比較検討を大規模レセプトデータを用いて行うことを目的としている。そのため、大規模レセプトデータ分析の条件設定を行うため、所属機関の院内レセプトデータを用いた入院医療費・外来医療の詳細分析を2022年度から行うこととしている。【当該研究に関する倫理審査およびデータの取得】採択通知後に、所属機関の倫理委員会に研究計画書を2022年7月15日に提出した。審査の結果、9月1日に研究計画の実施の承認をえた。その後、所属機関のデータ管理室より、院内レセプトデータ（DPCデータ）を2012年度～2021年度分までを2022年10月に取得した。【研究環境の整備】院内レセプトデータを扱うため、セキュリティレベルの高い状況で研究を行うため、研究環境の整備を行った。具体的にはスタンドアローンのPCおよび周辺機器（モニタ、入力装置）を購入した。【情報収集、他】他施設の研究者からオンラインミーティング等を活用し、データ分析についての助言をえた。【分析状況】院内データをPCに取り込み、データベースのクリーニングおよびスプレッドシートの作成中である。該当疾患は月0～10件ほどとなる仮計算の結果が出ている状況で、データが出そろったところで検算を行い、具体的な医療費の算出を行っていく前段階である。
