项目摘要

医療的ケアが必要な障がい児者が増えている。日本では、日常的に医療行為が必要な患者のケアとして「医療的ケア（以下：医ケア）」の枠組みがあり制度が進んできたが、医ケアの中心的担い手は家族介護者（以下:家族）である。申請者はこれまで医ケアが必要な重度障がい児者と家族について研究してきた。家族によると「夜間の訪問職員の人手が十分でなく、昼間は仕事、夜は医ケアの状態で休まらない」、「訪問看護師とともに子どもを入浴させるときに子どもを見てもらい、自分も手短に入浴を済ませている」など、日常生活援助に加えて医ケアがあることで、看護師や介護士を活用しても余るほどのケアと、それに伴う管理や観察が増大していた。また在宅障がい児者への全国調査を実施し、医ケア家族への負担の程度について解析した結果、他の要因の影響を調整しても、複数の医ケアを行う家族の介護負担は、医ケアのない家族（日常生活援助のみの家族）に比べて2.08倍(調整オッズ比):[95％信頼区間1.04-4.17]と顕著であった。2021年の医療的ケア児支援法では家族支援が重視されており、医ケア児者と家族の双方をケアする看護職への期待が高まっているが、家族への看護は十分ではない。看護職は、介護負担の側面のみならず家族の総合的なケア力を把握した上で、的確に医ケアへの支援を行う必要がある。そこで本研究は医ケア児者家族のケア力に着眼した新たな家族支援指標の開発を目指し、基礎調査（研究1）とアセスメント指標の作成（研究2）を目的としている。令和4年度は基礎調査の準備として、先行研究より、家族のケア力に関連する尺度の収集や質問紙作成を行った。研究協力者とともに質問内容の検討を行い、研究計画書を作成した。現在、所属大学の倫理委員会の審査を受けている。同時に、まずは京都府下の対象者を選定するため、事業所等の研究協力依頼先のリストを作成した。
