1. 研究目的本研究では障害児・者(疑いも含む)やその家族が乳幼児期から成人期までの一貫した支援を実現する「協働型発達支援システム」を確立するために、各自治体が持つ課題を整理し、社会資源や人的資源を有効に機能させるための政策的な提言を行うことを目的に、質問紙調査ならびにヒアリング調査を実施した。2. 研究方法関西の194市町村の母子保健分野ならびに教育委員会を対象に質問紙調査を実施した。質問内容は、相談支援体制、連携協議会やサポートファイル、保護者支援等についての現状と課題、課題への対策についてであった。ヒアリング調査は発達支援で先進的な取り組みを行っている2市町村(仙台市・泉南市)を対象とした。3. 調査結果調査票の回収率は母子保健分野34.5% (67/194)、教育委員会18.6% (36/194)であった。就学前から一貫した窓口を設置している市町村が16市町村(内滋賀県が9市町村)であった。連携協議会を設置している市町村は母子保健分野では88.0% (59/67)、教育委員会8.3% (3/36)であった。サポートファイルを運用している市町村は母子保健分野では61.2% (41/67)、教育委員会11.1% (4/36)であった。自由記述では、中学校卒業後に発達障害、不登校などの相談の受け皿が十分でないため、自立支援協議会の活用や相談対象年齢の引き上げなどを検討していた。この他、虐待や養育問題等の家族支援などが課題としてあげられた。4. 研究成果中学校までの支援体制は一定整備されてきているが、中学校卒業後の支援体制を確立していくことが課題となっている。対策としては滋賀県を中心に進められている乳幼児期から成人期までを対象とした課の創設、現在ある相談支援の対象年齢の見直し、専門職の常勤化を進めることが必要である。また養育問題を抱える家庭へ対応するため、各機関の連携強化や保護者支援の拡充(レスパイトや子育て技術を学ぶ機会の提供等)を行うことが重要である。

1.
