1.研究の目的医学部学生医療面接・診察能力訓練の為に,全国規模で客観的臨床技能試験(以下,OSCEと略)が実施されている。その際,被験者は実際の患者ではなく,模擬患者(以下,SPと略)が起用される。本研究では,「模擬患者自身の対応標準(平準)化」に焦点を絞り,学生の臨床技能教育及び学習到達度評価の質的向上に寄与させる事を狙いとして、OSCE標準化・客観化に寄与できる,課題提示者・模擬患者間でのシナリオ理解度平準化フォーマット(フォーム)のあるべき姿を策定・開発する事を目的とした。2.研究方法参考文献や研究者自身のこれまでSPを通しての業務経験から、SP自身が演じる内容を読解・学習する過程には,一定の特性・優先順位があるものと仮定し、それらを「訴える症状」・「症状の詳細」・「主観と客観に区分した病状の時系列変化」・「患者を取り巻く背景」・「気になっている事」・「患者自身が病気についてどう思っているか」・「望む(嫌な)治療」の各カテゴリに分けて表記する理解度平準化フォーマットを考案し、平成22年度末、実際のSP養成講座で試行評価を行った。3.結果試行の結果、本平準化フォーマットの活用により、SPのシナリオ習熟までに要する育成絶対時間の短縮(当初3時間×2回を、約2時間×2回に短縮)と、SP全体の理解度平準化に一定の効果が見られた。また、試行に参画戴いたSP各位に集合調査法にて行ったアンケート結果も、概ね良い評価・回答内容であった。4.後述、今後の展開今後、本フォーマットの更なるブラッシュアップを図ると共に、SPシナリオ習熟に係るプロセスのマニュアル化を進め、SP育成を始めとするOSCE全体の評価平準化・業務合理化に寄与していく。また、今回の活動内容・成果について、平成23年度、関連学会に学術論文の投稿・発表を予定している。
