目的:腎移植患者におけるバルガンシクロビル投与量とバルガンシクロビルの活性代謝物であるガンシクロビル血中濃度の変動を追跡し、腎移植患者におけるガンシクロビル血中濃度と抗ウイルス効果および骨髄抑制の副作用との関連性について検討することを目的とした。方法:バルガンシクロビルが投与された腎移植患者を対象とし、経時的な採血を実施し、ガンシクロビル血中濃度は高速液体クロマトグラフィーを用いて測定した。採血は、バルガンシクロビル投与開始後3日目以降の投薬前と投薬2時間後に行い、投薬期間中、1又は2週間に1回採血を行い、ガンシクロビル血中濃度測定を行った。バルガンシクロビル投与量、サイトメガロウイルス(CMV)アンチゲネミア、白血球数、好中球数、ヘモグロビン、血小板数、血清クレアチニンの値を、病院内オーダリングシステム臨床検査データベースから調査した。本研究に関する臨床研究実施計画は、鹿児島大学病院臨床研究倫理委員会で承認を得ている。結果:対象患者は4例(男性2例、女性2例)で、CMVアンチゲネミア陽性化時にバルガンシクロビルの投与を開始し、陽性細胞数が陰性化したら中止された。4例のバルガンシクロビル投与量は、添付文書に記載されたクレアチニンクリアランスに基づく推奨投与量の1/2量であった。4例のクレアチニンクリアランスは、29.7～63.4mL/minであり、CMVアンチゲネミア陰性化時のガンシクロビル血中濃度(トラフ値)は、0.38μg/mL、0.47μg/mL、0.54μg/mL、1.80μg/mLであった。投薬2時間後のガンシクロビル血中濃度の平均は4.24μg/mLであった。骨髄抑制の副作用の指標として白血球数、ヘモグロビン、血小板数に変動は認められなかった。考察:腎移植施行患者において、推奨投与量よりもバルガンシクロビルを減量した投与方法でCMVアンチゲネミア陰性化が得られ、患者個々のガンシクロビル血中濃度データを得ることができた。
