项目摘要

リエゾン精神医学では身体科入院中の患者の精神的問題に対処するが、その他に、患者一家族および医療者関係、1医療スタッフ間の葛藤などに関与するため、精神科医はさまざまな倫理的問題に直面する。そこで平成16年度には、リエゾン精神科医が感じた倫理上の問題と対応に関する実態を調査した。その結果、倫理的問題の内容として、インフォームド・コンセント、精神症状による身体的治療・検査の阻害、身体的拘束、鎮静、安楽死などに関わる問題が挙げられた。平成17年度にはこれらの結果を基に調査用紙を作成し、総合病院に勤務する精神科医(103施設、129名)に倫理上の問題に関するアンケートを実施した。その結果、リエゾン活動において精神科医は倫理的葛藤を生じ得る状況をしばしば経験していることがわかり、その状況は上記の他、患者のプライバシー、精神症状による患者の判断力、予後告知などに関わる状況であった。平成18年度には、このような倫理的な葛藤状況を他の医療スタッフに伝える方策を試みた。多忙の中、精神科リエゾン医と身体科医療スタッフとの間で十分な意見交換が出来ない場面も多く、その改善を目的に、精神症状の推移および倫理的問題の有無とその変化がわかるチェックリストを作成し、通常の回答書に添付し報告することを試みた。2つの総合病院精神科に調査を依頼し各6週間実施した。新患33名、再診44名、延べ115名が対象となった。その結果、倫理的問題が存在したものは4例で、その中には当初身体科スタッフには気付かれていなかった倫理的問題がチェックリストの利用により改善が図られたものがあった。一方、倫理的問題が挙がったものは77例中4例と少なく、回答書にチェックリストを常に添付することの問題が指摘された。但し、このようなチェックリストを用いることで倫理的問題が改善された例もあり、今後、電子カルテ化される中で指摘された問題は解決され得るものと考えられる。
