项目摘要

肢体不自由児施設において、通園児の母親に対して聞き取り調査及び卒園(小学校就学前)・退園時の両親の手記218名を分析した。母親からの手記が213編に対し父親は5編にすぎず、医療関係者からの聞き取り調査においても父親の障害児の受容は、母親以上に困難な場合が多く、父親が母親を支えるのではなく、母親の受容過程を妨げていると思われる事例もみられた。出生直後の病院での対応が児の受容を妨げていることを疑わせる事例も見られた。母親を看護するスタッフとNICUスタッフとの情報交換・連携が適切になされ、両親の障害児の受容過程を促す看護が必要である。イギリスでNICUに母親(褥婦)が入院するというシステムの施設を訪問した。新生児を取り巻く家族あるいは母親を取り巻く新生児を含めた家族として家族単位で対応することが医療関係者には不可欠であることを学んだ。母親が精神的障害をもった場合、我が国では母親は精神疾患治療施設に入所し、児と母子分離が避けられない現状であるが、イギリスでは、精神障害を持つ母親と児が同一のユニットに入院できるシステムが確立されているなど家族をひとつの単位として対応する必要性を提言したい。胎児や新生児になんらかの異常が疑われる場合の告知についても考慮する必要性が示唆された。児の予後について、児の可能性を否定するように受け取られる表現ではないか、告知場所についても静かな穏やかな環境が整えられた場所であるのかなど目細やかな配慮が必要である。告知された父親などへのサポートも重要である。本研究により、障害児の受容は、児の障害の程度に比例していないことが示唆された。母親の育児感や生育歴に加えて、父親やその他の家族のサポート体制、専門家の支援、レスパイトケアなど社会的支援により、受容過程は異なると考えられる。障害を有する児の育児家族への援助に本研究結果をいかしていきたい。
