项目摘要

問題行動が発生する過程に、どのような認知が影響しているのかを把握するため、攻撃性に関する質問紙調査を実施し、対象少年の特徴をより明らかにするために、公立小・中学校に通学する同年齢の児童・生徒315名と比較した。その結果、「攻撃的問題解決方略への有能感」と「個人的欲求への固執」において有意差がみられ、児童青年精神科に入院する児童に高く示された。また、公立小・中学校に通学する児童を、攻撃性を示しやすい者と示さない者に分け、入院児童と比較したところ、攻撃性を示しやすい者-示さない者間で有意差がみられた「報復意図」、「攻撃行動への肯定的評価」が入院児童ではみられず、「攻撃的問題解決方略への有能感」がどちらの群とも有意差を認めた。その後に実施した児童青年精神科思春期病棟での参加観察でも問題行動は、思うようにならない場面で高圧的に振る舞ったり、相手の気持ちを無視して進める状況で多く発生していた。問題行動の件数は前回に比べ減少していたが、入院児童の構成が、(1)小学生中心から中学2・3年生中心に変化したこと、(2)症状として攻撃性を示しやすい児童の入院が減少したという物理的な要因以外に、職員の対応が指摘・注意といった否定的なものから気づきを促し子ども自身に考えさせるもの変化したことも要因と考えられた。質問紙調査・参加観察の分析から、対象少年は自らの欲求を充足するために「攻撃」を用い、相手に言うことを聞かせることで安定を得るという特徴があることが明らかになった。こうした少年が力関係に頼らなくても自己主張できるように、少年個人を尊重し、行動の是非や相手の気持ちに気づかせるようなアプローチすることの必要性が示唆された。初年度の結果を国際学会(ICN24^<TH> QUADRENNIAL CONGRESS 2009.6月)で発表する予定である。
