项目摘要

探索型臨床研究は、基礎医学で得られた成果に立脚し、治療法がない患者のために新しい治療を開発するものである。その遂行には公平性や透明性が必要となるが、脳死移植などの過剰報道やプライバシーの遵守逸脱に見られるように過度にこれを進めると社会の不信を招き、逆に開発医療を停滞させることになる。医療倫理はこれらのバランスを保ちながら、どのような立場から見ても合意の得られる医療が行われるようにすべきものである。探索型臨床研究における公平性と透明性を保つため、探索型臨床研究に直接的に関わらない医療スタッフをトランスレーショナルリサーチコーディネーター(TRC)として組織し、臨床研究の公平性と透明性の維持、患者権利の保護、データ管理等を含めて、臨床研究が円滑に行われるための試みを開始した。TRCは薬剤師、看護婦、臨床心理士、栄養士、臨床検査技師の各専門家で構成されている。業務内容は、(1)医療倫理の監視(患者への臨床研究内容の事前説明、説明同意への立ち会いとその公平性の確認)、(2)プロトコル遵守の監視(症例検討会議の出席、医師を交えて行う週1回のTRC会議)、(3)被検者のケア(病棟訪問、心理状態の把握)(4)書類管理である。医療倫理面での説明同意への立ち会いには3-4名のTRCが同席し、医師の行う説明同意の内容を10項目以上にわたり評価してスコアー化する。説明同意の後にTRCは医師にこれらを伝え、被検者の理解度が低かった部会については医師に追加補足するよう指示する。本研究ではこれまでに探索型臨床研究プロトコル、及び説明同意書のガイドライン作成における医療倫理遵守のための事項を確定し、実際にこのガイドラインの作成を終了した。また臨床研究開始前と開始後の被検者の定量的な心理分析も実施し、目標を達成している。米国と日本における医療倫理の相違点と共通点の解析については、米国のコーディネーター養成のためのテキストブックの内容をTRCとともに検討し、日本のTRC養成のためのテキストブックの医療倫理の項目を充実させた。

The TRC consists of various departments, including physicians, nurses, clinical psychologists, nurses, and clinical laboratory technicians. The business includes: (1) monitoring of medical ethics (prior explanation of clinical research content of patients, confirmation of fairness of consent and establishment meeting);(2) monitoring of compliance (attendance of case review meeting, doctor's communication, weekly TRC meeting);(3) monitoring of patients (patient visit, psychological status assurance);(4) book management. 3-4 TRC members in the Standing Committee of the Medical Ethics Committee and 10 or more items in the Medical Ethics Committee TRC physician's understanding of TRC approval is low and TRC physician's understanding of TRC approval is low This study is intended to determine and complete the medical ethics compliance issues related to the preparation of exploratory clinical research documents and the consent forms. Quantitative psychoanalysis of subjects before and after the start of clinical research was carried out to achieve the objectives. The analysis of the contradiction and common point of medical ethics in China and Japan has been carried out in China and Japan.
