The Prevention of Crimed Commited by Psychiatric Patients, Based on Forensic Psychiatric Evidence.

I have noticed psychiatric patients who commit crimes in spite of contact with medical facilities, through experiences of psychiatric evidence. So, the aim is to clarify problems of the psychiatric medicine and the administration of mental health service in Japan, and to discuss measures about them.At first, this paper reviews risk factors associated with crime, risk assessment and management, and systems to treat for psychiatric patients who have committed crimes. Next, it investigates 75 instances of forensic psychiatric evidence for the period of April 1997-September 1999, at the Group of Mental Hygene, Community Medicine, University of Tsukuba. It divides a subject into "Non contact "group and "Attend at a hospital and consultation" group, to pay attention to condition of contact with associated facilities (hospital, public health center and police) and compares the two groups. Over again, it divides the latter into "Discontinuation" group and "Continuation" group, and does also.In consideration of results and review, it finds that condition of contact with associated facilities does not relate with demographic data, patients who have committed severe crimes are likely to contact with them, schizophrenic patients are likely to contact with them, personality disordered patients are not likely to contact with them, mental retarded patients who need not to keep treatment repeat deviant behavior, substance-related disordered patients are not likely to continue to attend them, discontinuation with them relates with repeating to commit crimes and delusion of injury which schizophrenic patients have relates with crimes.But it cannot identify detailed problems of instances only with mass data and it needs particular analyses of them. So it will serve instances with dividing types and investigate detailed them, and discuss how to treat them and suggest the new system for treatment.
