In order to elucidate the effect of secondary hyperparathyroidism and the modulation of parathyroid function on the ectopic vascular calcification in uremic rats, aorta and arterial vessel of the 5/6 nephrectomized rats with severe secondary hyperparathyroidism fed by hyper phosphate and hypo calcium diet were examined. Remarkable ectopic calcification (von Kossa positive staining) was observed in the aorta of experimental uremic rats with secondary hyperparathyroidism, and at the calcified vessel, m-RNA and protein expression of sodium-dependent phosphate co-transporter (Pit-1), core binding factor-alfa 1 (CbFa1) and osteopontin (OPN) were observed. These findings depended on the severity of secondary hyperparathyroidism and hyperphosphatemia and serum calcium and phosphate products. These results suggest that secondary hyperparathyroidism per se stimulates vascular calcification, and promotes the active transport of phosphate into the vessel wall and develops ossification of vascular smooth muscle cell in uremia. Management of calcium, phosphorus and secondary hyperparathyroidism is very important to prevent the vascular calcification and associated atherosclerosis.
