本研究の目的は、プロスタグランジン生成に関わる酵素および転写因子の遺伝子多型と大腸がんとの関連を明らかにすることである。プロスタグランジンは、リノール酸の代謝産物アラキドン酸を基質としてサイクロオキシゲナーゼ(COX)により生成されるが、細胞増殖、アポトーシス抑制、血管新生などの機序を介して大腸発がんを促進すると考えられている。COXには非誘導型のCOX1と誘導型のCOX2があるが、大腸発がんにはCOX2がより重要であると考えられている。炎症性サイトカインは転写因子NFκBを介してCOX2遺伝子を発現させる。本研究では、大腸がん症例685例と住民対照778例について、機能が確認されているCOX2遺伝子多型-765G>C(dbSNP ID rs20417)と大腸がんリスクとの関連が指摘されているpolyA近傍の多型9850A>G(rs4648298)およびNFκBIAの3-UTR多型(G>A)を検討した。COX2遺伝子多型-765G>Cおよび9850A>Gの変異型アレル頻度は対照群でそれぞれ0.45%および3.0%であった。これらの変異型アレルの頻度は欧米で報告されている頻度に比べきわめて低率であり、ホモ変異型は症例群および対照群で皆無であった。ホモ野生型に対するヘテロ変異型のオッズ比は-765G>Cで0.9(95%信頼区間0.6-1.5)、9850A>Gで1.0(95%信頼区間0.3-3.0)であった。NFκBIAの変異型アレルの頻度は高率であったが(31.3%)、NFκBIA多型と大腸がんリスクとの関連は見られなかった。ホモ野生型GG型に対するGA型およびAA型のオッズ比はそれぞれ1.0(95%信頼区間0.8-1.2)と1.2(95%信頼区間0.9-1.8)であった。
