项目摘要

本研究の目的は、大腸がんの症例対照研究(症例840名、住民対照833名)において収集された調査資料とDNA試料を用いて、大腸発がんにおける食物栄養要因の役割を明らかにすることである。食物繊維及び大豆イソフラボンと大腸がんリスクとの関連を検討し、合わせてP53とその発現規定分子MDM2の遺伝子多型解析をおこなった。食物繊維摂取量と大腸がんリスクとの関連はみられなかったが、食物繊維の主要摂取源である米飯が大腸がんに予防的であった。米飯摂取と大腸がんリスクとの関連についてはこれまでに定量的な検討はなされていない。一方、大豆イソフラボン摂取量は男性及び閉経後女性の大腸がんリスク低下と関連していた。P53はがん抑制遺伝子であるが、機能的多型TP73R72Pの大腸発がんにおける役割は定かでない。MDM2はP53に対して抑制的であり、機能的遺伝子多型SNP309(T309G)の発がんあるいはがん進展への関与が注目されている。遺伝子解析の同意が得られた大腸がん症例685例と住民対照778例についてこれら2つの遺伝子多型と大腸がんリスクとの関連を検討した。男性でのみTP53-72Pアレル及びSNP309Gアレルと関連した大腸がんリスクの統計学的に有意な高まりが観察された。また、TP53-72PアレルとSNP309Gアレルを同時に有する男性では、2つ多型がホモ野生型である男性に比べて大腸がんリスクの2倍の高まりがみられた。女性ではこのような関連はみられなかったが、この理由については検討が必要である。
