项目摘要

The purpose of this study was to investigate the effect to the human body functions with correcting the mandibular position. We evaluated the 3D asymmetry of faces, the microcirculation of the blood flow of fingers, the heart rate variability calculated from the blood pressure fluctuation, the skin surface temperature of fingers, the grip strength, and the body anteflexion on standing, in TMD patients with the mandibular deviation and normal subjects.The results were summarized as follows :1. The 3D asymmetry of faces in the patients is severer than that in normal.2. When the mandibular position of normal subjects was changed, the microcirculation of the blood flow of fingers was changed after moving necks.3. The LF/HF ratio of spectral analysis for the heart rate variability in the patients was significantly lower than in normal. It was suggested that the cardiovascular regulation of the patients was damaged. In the patients, the LF/HF ratio after correcting the mandibular position was significantly higher than before, and it was close to the ratio in normal.4. Correcting the mandibular position of the patients made the skin surface temperature of the fingers rise in the right hands in 4 of 6 and in the left hands in 3 of 6, and fall in the right hands in 0 of 6 and in the left hands in 2 of 6.5. Correcting the mandibular position of the patients made the grip strength increase in the handedness in 13 of 18 and in the other hand in 9 of 18.6. Correcting the mandibular position of the patients made the rate of the body anteflexion on standing increase in 11 of 16.From the results, it is suggested that the body functions are influenced by the mandibular deviation.
本研究旨在探讨矫正下颌骨位置对人体机能的影响。本研究对伴有下颌偏斜的TMD患者和正常人的面部三维不对称性、手指血流微循环、血压波动计算的心率变异性、手指皮肤表面温度、握力和站立时身体前屈进行了评价。患者的面部三维不对称性较正常人更明显.正常人下颌位置改变时,颈部运动后手指血流微循环发生改变.患者心率变异性频谱分析的LF/HF比值明显低于正常人。提示患者心血管调节功能受损。患者下颌位矫正后LF/HF比值明显高于矫正前,接近正常值.矫正下颌位后,6例患者中4例右手手指皮肤表面温度升高,6例患者中3例左手手指皮肤表面温度升高,6例患者中0例右手手指皮肤表面温度降低,6例患者中2例左手手指皮肤表面温度降低。矫正患者的下颌位置使18名患者中13名患者的惯用手握力增加,18.6名患者中9名患者的另一只手握力增加。16例患者中11例在矫正下颌骨位置后,站立位前屈率增加。提示下颌偏斜对身体功能有一定影响。
