矯正治療は歯周疾患の発生の予防、治療に有効な手段であるが、矯正治療が歯周組織にとって時として有害に作用することがある。本研究では、矯正治療による歯周管理効果を判定することを目的として歯列不正や咬合異常をきたした中等度以上の歯周炎罹患者を対象に、矯正治療中の歯周組織の変化を臨床的パラメ-タ-及び歯肉溝滲出液の量的、質的な変化でとらえ検索した。被検者は本学附属病院保存科を受診した2名の歯周炎罹患者を使用した。歯肉溝滲出液での質的評価として、コラゲナ-ゼ活性、酸性フォスファタ-ゼ及びハイドロキシプロリンを測定した。その結果:矯正装置装着前後におけるG.I.、P.L.I.、G.BIポケットなどの臨床的パラメ-タ-は低い値を示し大きな変化はみせなかったが、1名の患者において歯肉溝滲出液はPeriotron値で290から835と増加した。それに伴い炎症マ-カ-の酸性フォスファタ-ゼ活性も30から85K-Aunitと上昇した。さらに、初期治療終了、矯正装置装着前の潜在型、活性型コラゲナ-ゼの比(L/Att)が3.00であったものが、1.87と減少した。ハイドロキシプロリン量も、この患者では254ngより382ngと増加していた。一方、他の患者では酸性フォスファタ-ゼ活性、L/A比並びにハイドロキシプロリン量は、矯正装置装着前後で大きな変化はなくしかも、歯肉溝滲出液量も前後において差は認められなかった。このことは、歯肉溝滲出液の量的、質的変化の評価は臨床的パラメ-タ-ではとらえることのできない歯肉の炎症性変化を鋭敏に反映することを示唆する。さらに、矯正治療中の歯周管理の上からも、治療効果、予後の判定に十分に応用できるものと考えられる。
