総義歯の治療効果を得るには,床下粘膜組織の健康が重要であり,新義歯製作に先立ち変形や病変を伴った粘膜の回復を目的として,粘膜調整材が使用される.しかし,粘膜調整材は材質の経日的変化を考慮せず長期間使用されることも少なくなく,また長期間の使用が顎口腔系の機能に及ぼす影響もいまだ明確ではない.そこで,粘膜調整材の長期使用による材質の烈火が咀嚼機能に及ぼす影響について以下の実験を行った.(方法)被検者は上下顎義歯を製作し,下顎位ならびに咬合接触関係は良好であるが,下顎総義歯のリライニングが必要と判断した男性3名,女性2名を選択し,使用中の下顎総義歯に粘膜調整材を使用し,カマボコおよびピ-ナッツを主咀嚼側にて咀嚼した時の咀嚼運動経路と咀嚼筋筋電図(被検筋:左右側側頭筋後部および咬筋)を同時記録した.観察期間は粘膜調整材使用前,使用直後,1日後,3日後,1週間後,2週間後とし,測定結果は分散分析法にて統計学的に検討した.また粘膜調整材使用義歯に対する被検者の主観的評価の推移に関して,VAS法を用いたアンケートを行い装着感と合わせて検討を加えた.(結果)1.咀嚼運動経路は,1週間以後,開口量ならびに側法偏位量が有意に減少した.2.咀嚼筋筋電図の分析より,1週間以後,筋活動のリズム性,ならびに筋活動量が有意に低下した.3.咀嚼筋筋電図による群化放電分析より,経日的に随意性の強い咀嚼運動へ移行した.4.被検者の主観的評価(装着感)は,良好なまま推移した.以上より,粘膜調整材の長期使用は患者の主観的評価に変化を与えなくても,咀嚼機能の律動性を低下させることが示唆された.今後,粘膜調整材の理工学的性状の変化を加えることで,より詳細に粘膜調整材の咀嚼機能への影響関係が明かになるものと思われる.
