日本の医師患者関係において家族が果たす役割について、医師・患者・家族へのインタビュー調査をとおして探った。現時点では以下まで終了した。1.文献収集・分析:告知についての患者・家族への意識調査、手記、医療倫理関係・家族社会学関係の文献を分析し、患者家族の意見の全般的傾向、医師・患者・家族のお互いへの役割期待、医療倫理学での家族での家族の捉え方、社会学的にみた家族間の意志決定のあり方、医療・看護・介護に関わる家族関係の現状を探った。2.患者会において参与観察:患者から見た情報の流れ、家族との関係、役割期待について予備的な理解を得た。3.インタビュー調査の準備・実施:協力医師とのうちあわせ、対象者の決定、インタビュー内容の決定を経て、患者、家族、医師、計25回実施した。医師からの患者・家族への情報提供の内容、各人の情報把握の程度、医師・患者・家族間の役割期待や自己の役割認識について探った。インタビュー内容のテープ起こし。4.データ分析:現在、テープおこしが終わったものから、順に分析を行っている。情報の流れの具体的な把握に関しては予想以上に医師・患者・家族の3者間でずれが大きかったが、互いへの役割期待、自己の役割認識については共通点が多かった。家族は患者に対し、医師と共に保護的役割を演じる傾向が強いが、患者はそれに葛藤を感じつつも受け入れざるを得ないという態度が多かった。医師は治療の遂行のため家族に様々な役割を要求していたが、家族もそれを引き受ける姿勢が強かった。家族の役割の矛盾する内容と情報の流れの関係については、今後の分析に待ちたい。
