要介護高齢者を介護する家族の負担を軽減していると思われる要因の探索のために,まず28ケースの家族介護者に対して詳細な面接調査を行い,軽減要因の抽出を行った。これらの家族介護者は,一総合病院を退院した脳血管疾患後遺症を持つ高齢者を介護している介護者で,かなりたいへんな世話を必要とするケースであった。さらにそれら抽出された軽減要因の概念的整理を行って,20項目の質問文を作成した。負担の軽減要因として質問文に包含された事項には,介護のための情報収集,計画的対応,介護者自らの健康管理,各種サービスの利用,専門家への相談,家族の協力といった介護者自らの問題解決型の軽減要因,および,周囲からの情緒的支持,感情表出の機会,気晴らし行動,行動の正当化といった認知変容型の軽減要因であった。この20項目の質問文を用い,家族介護者に対して配票留置回収で調査を実施した。調査対象は,東京都下A市在住の要介護高齢者を介護する家族介護者536名で,なんらかの形で行政が把握している家族介護者であった。負担の軽減要因に加えて,軽減要因との関連を探索する目的で,要介護高齢者の身体状態および精神状態,主・副介護者の年齢・就労状況・介護期間・介護程度,主介護者の身体的・精神的・社会的負担,保健福祉サービスの利用状況,主介護者への社会的支援などの測定を同時に行った。負担の軽減要因は,要介護高齢者の身体状態および精神状態,介護者の経験している負担,保健福祉サービスの利用状況と関連があることが判明した。

The burden of family mediation should be reduced, and the important factors should be explored and investigated in detail.
