本研究は、高齢者での社会的ネットワークと生命予後との関連に、健康状態の客観的側面が交絡要因として及ぼす影響の大きさを、主観的側面のそれと比較することを目的とする。解析対象者は、1991年3月に高知県大月町で、65歳以上在宅高齢者を対象として実施した、生活様式と健康状態に関する悉皆質問紙調査に回答し、かつ1990年度に同町で実施された老人保健法による基本健康診査を受診した784名である。このコホートでの1995年3月31日までの死亡者54名を、住民基本台帳との照合により同定した。解析対象とした社会的ネットワークは、就労・役割、婚姻状況、世帯構成、子との交流、親族との交流、友人との交流、近所づきあい、グループ活動参加の8項目であった。健康状態の主観的側面として自覚的健康状態を、客観的側面として基本健康診査所見の総合判定結果を利用した。死亡との関連を示す相対危険度の点推定値が2以上であった社会的ネットワークの項目は、男の65-74歳群では、世帯独居、配偶者なし、グループ活動参加なしの3項目、男の75歳以上群では、友人なし、グループ活動非加入の2項目、女の65-74歳群では、就労・役割なし、友人なし、隣人との交流なしの3項目、女の75歳以上では、隣人との交流なし、グループ活動非加入の2項目であった。Mantel-Haenszel法を用いて自覚的健康状態を調整すると、これらの相対危険度の多くが減少し、特に女の75歳以上群でのグループ活動非加入の値は53%も減少した。一方、健康診査の総合判定結果を調整した時の相対危険度の減少は、最も減少が大きかった男の75歳以上群での友人なしとグループ活動非加入でも13%にとどまった。従って、健康状態の主観的側面と客観的側面とでは、社会的ネットワークと死亡との関連を交絡する状況が異なっており、どちらか一方のみの調整では十分でないと考えられた。今後、生存機関を考慮するCox比例ハザードモデルを用いた検討を進める。
