项目摘要

老人介護施設の数施設の介護職員に対し、精神的健康度に関する施設環境としての問題点を聴取し、職種による職場環境の特徴、規模や入所者属性、地域による差などについて考察した。とくにグループホームや小規模介護の施設などに対し、施設環境の違いによる差を考察した。また、施設入所者の行動調査をあわせておこなった。小規模化、個室化の進んだ施設においては、総じてケアの方法が積極的で意欲的であり、動線の長さなどの労働上の問題点もあるが、個別ケアをおこなう上での精神的な満足感や達成感が強いことがわかった。施設職員の精神的健康度を捉えるにあたっては、まずその指標化をおこなった。医師や看護婦、教師などに対しこれまでなされてきた既往研究による諸指標を参考に、老人介護施設職員用の精神的健康度として、日常的なストレス、モラール尺度、燃え尽き症候群スケール、生活満足度などの心理的尺度を採用した評価項目を作成した。この精神的健康度の尺度により、特別養護老人ホーム施設職員に対する調査をおこない、とくに、生活指導員と施設長、寮母の職種による差が認められた。さらに、施設規模によっても若干の差が認められ、小規模介護の有効性がうかがえた。施設形態に特色のあるグループホームなどの施設環境の形態による影響を考察したが、現在のところ、少数例でかつ特殊例の扱いであるため、尺度による数値の明確な差異として結果を断定するには至らなかった。
