食道癌術後患者に,術後の嚥下感覚の変化の有無,術後の嚥下感覚の変化に対するインフォームド・コンセント(以下IC)の有無,患者が術後に嚥下感覚の変化をどのように認識しているか等について,半構成的質問用紙を作成し,10名の患者にインタビューを実施した。同時に,食道癌術後の嚥下状態について評価するために,13名の術後患者に対して,頸部電気インピーダンス(以下IPG)および嚥下音測定を実施した。インタビューの結果,患者は術前のICにおいては,疾患名,術式,術後の状態などについて詳しく説明を受けていたが,術後の嚥下感覚の変化については説明されておらず,術後,嚥下感覚の変化を自覚した時点で,驚きと不安を感じていた。しかし,その認識の仕方は患者によって差があった。また,IPG及び嚥下音測定結果を青年健常者の測定結果と比較してみると,食道癌術後患者では,IPG波形の多相化,嚥下音聴取時期のズレが認められた。IPG波形の多相化から,食道癌術後患者は,一度口に入れた食物を,一度で飲み込み切れておらず,何度かに分けて飲み込んでいると考察された。また,嚥下音が,咽頭期以外の時期で聴取されていることから,咽頭を食物が通過するときに,気管入口部が完全に閉鎖されていないことが考察された。このことから,誤嚥を起こす可能性が示唆された。以上より,食道癌術後患者には,嚥下活動自体に変化が起こっていることが明らかとなった。また,患者は術前のICにおいて,術後の嚥下感覚の変化について説明を受けておきたいと希望していることから,担当医師との調整を行い,説明の中に術後の嚥下感覚の変化が起こる可能性についての説明を加えている。術後の嚥下機能の変化,その感じ方は患者個々によって異なることより,今回の研究で有効なリハビリテーションを考察することができなかった。今後は,IPG及び嚥下音測定を経時的に実施し,嚥下機能の経時的変化を追跡すると共に,患者がどのようなプロセスを経て新しい嚥下方法を身につけているかについて,インタビューを継続して行いながら,有効なリハビリテーションについて考察していきたいと考えている。

In the future,IPG and swallowing sound measurement time implementation, swallowing function of the time change tracking, together, patients can change from the beginning to the end of the new swallowing method in the body, there are many aspects of the study.
