前年度までに、要介助高齢者用ADL(日常生活動作)能力指標(ADL-PDI)を寝たきりおよび自立高齢者に対して実施し、要介助高齢者のADL評価に有効な動作のうち、要介助高齢者が寝たきり状態に陥る際または、要介助高齢者が自立する際に重要な評価項目を明らかにすることを試みた。横断的評価の結果、機能水準判別に関する以下の知見が得られた。1)ADL-PDIは難易度の異なる17項目から構成されており(17点満点)、項目間の難易度に基づく一次元性が保証されていること、2)本指標の総合得点が5点以下の場合または、難易度の低い下位4項目(食事、トイレへの移動、寝返りする、字を書く)が成就不可能な場合、寝たきり状態に陥る危険性が高いこと、3)総合得点が13点以上の場合または難易度の高い上位5項目(ズボンをはく、ゴムズボンをはく、座位から立ち上がる、階段を上る)が成就可能な場合、自立水準に近い状態にあること。本年度、上記の横断的評価から得られた知見が、縦断的評価においても有効か否かについて検討した結果、一定期間前後の縦断的評価時にも有効と考えられた。つまり、ADL-PDIによる評価から一定期間後の高齢者の機能水準をある程度予測する際の有効性が示唆された。縦断的な機能変化を評価する際の感度について、既存の障害高齢者用ADL指標であるBarthel Index(BI)と比較した結果、機能水準の低い障害高齢者に対してはBIの方が優れるが、ADL-PDIは機能水準の比較的高い障害高齢者にも適用可能であり、適用範囲がBIよりも広範囲である傾向が認められた。ただし、本研究は限られた標本でのみ行われていることや、縦断的な追跡期間も短いことから、今後これらを考慮したうえで縦断的評価の有効性についてさらに検討する必要がある。
