本研究では、筆者のこれまでの研究(横断的資料に基づくADL指標の有効性およびADL能力特性)で得られた知見をもとに、1)定期的なADL評価を行った際に、既存のADL指標が機能特性の変容をどの程度評価値に反映しうるか、2)要介助高齢者のADL能力における縦断的な能力特性の変容にはどのような特徴が見られるか、3)横断的資料に基づいて筆者がこれまでに提案した機能水準判別のスクリーニング基準が縦断的資料にも反映されるか、について検討してきた。本年度は定期的な各種リハビリテーションが要介助高齢者の動作能力特性に及ぼす影響や、我々が提案した高齢者機能水準判別スクリーニング基準の縦断的評価時の有効性について検討した。養護老人ホームに入所する62歳から88歳の高齢者12名(男性4名、調査開始時点72.5±9.9歳、女性8名、75.9±5.7歳)を対象に8〜12ヶ月間の縦断的な調査(ADL、自立度、リハビリテーション状況等)を実施(毎月1回)した。調査期間中、各被験者は定期的なリハビリテーションを実施した。調査期間中、ADLの機能特性が変化しない者と変化した者が認められた。変化が認められた者における機能変化の特徴には個人差が認められたが、リハビリテーションにおける安定期段階にある者が多かったため、基本的に著しい機能の改善は認められなかった。食事動作やトイレ動作など、従来の横断的調査において比較的難易度が低かった動作が改善する傾向が認められた。また、Barthel indexと比較して、我々の要介助高齢者用ADL指標の方がADL評価に経時的な変化が観察された。筋力発揮調整システムも含めた各種リハビリテーションを伴う要介助高齢者の縦断的な機能評価について施設入所者を対象に検討したが、今後、リハビリテーションの段階をより詳細に踏まえた実験計画に基づいての検討が必要である。
