项目摘要

介護老人保健施設において痴呆性高齢者を対象に園芸療法を行なった。これまでの研究結果をふまえ、対象者は園芸や農業の経験がある入所者とし、今年度はプログラムの中に、能力はあるが施設入所に伴いできなくなった作業や、栽培した植物の加工等を積極的に取り入れ、各セッションには複数のプログラムを準備し、参加者が選択できるようにした。セッションの始めには、前回の活動時の写真を使って振り返り、さらに参加者全員が、五感を通じて植物の成長を確認できるよう促した。活動の評価は昨年と同様、各対象者の活動目標の到達度等を、個別に、質的に評価した。セッションを重ねる中で、参加者が誘い合って同じプログラムに参加し、協力して作業を完成させる場面がみられ、人物に関する認識が高まった可能性が考えられた。五感を通じて植物の成長を確認する中では、過去の栽培の記憶や、植物の成長を見守るような言葉が聞かれた。また、栽培した野菜を使った浅漬け作りと試食については、その後も長期にわたり記憶が保持されており、五感を刺激する作業は、認知機能や記憶の保持に効果的であると考えられた。さらに浅漬け作りでは、対象者が包丁を使って野菜を刻んだが、手馴れた様子で作業を行なった。このような、能力はあるが施設入所に伴いできなくなった作業を取り入れることは、対象者の能力を的確に判断し、看視下でということが前提ではあるが、在宅復帰に向けての能力の維持という点で、効果的であると考える。また別の対象者は、野菜は刻まずに、刻んだ野菜をもみ込む作業を自ら希望した。活動中、常に対象者自身が選択する機会を作ったことで、自分自身で考え、意思を表出できるようになった可能性が考えられた。今後は、さらに在宅復帰後のQOLの向上に寄与すべく、園芸療法を通じて、入所者の家族への働きかけを積極的に行ない、在宅復帰後の環境作りにも取り組んでいく必要があると考える。
