1.肝内胆管癌におけるラミニン5、オステオポンチン発現の意義ラミニン5は分化型胆管癌もしくは、胆管内発育型では基底膜側の発現を認めるが、肝実質に浸潤して形成された腫瘤形成型では腫瘍細胞と周囲の腫瘍間質にも発現していた。ラミニン5の腫瘍間質での発現は予後不良であった。また、オステオポンチンは腫瘍細胞の運動能を促進すると言われていたが、胆管癌の内腔側に発現し、分化度と正の相関を示し、その発現低下は予後不良であった。2.肝内胆管癌における胃型粘液発現の意義肝胆膵領域の癌においては癌化に伴う粘液異常発現が報告されているが、胃型粘液MUC5AC、MUC6に注目し肝内胆管癌の腫瘍発育、予後についての検討を行った。肝内胆管癌外科切除100例を対象としてMUC5AC(45M1,CLH2)、MUC6(CLH5)、MUC2(Ccp58)を用いた免疫組織化学的染色を行い(陽性癌細胞10%以上を陽性例と判定)臨床病理学的に検討した。MUC5ACは2種類の抗体ともに40%に陽性で、MUC6は20%、MUC2は9%に陽性であった。MUC5ACとMUC6の染色結果により対象はMUC5AC(-)/MUC6(-);null type(n=43)、MUC5AC(+)/MUC6(-);gastric foveolar type(n=36)、MUC5AC(-)/MUC6(+);pyloric gland type(n=11)、MUC5AC(+)/MUC6(+);gastric combined type(n=10)の4群に分類され、MUC2陽性の9例は、gastric foveolar typeとgastric combined typeにのみ認められた。gastric foveolar typeとgastric combined typeは肝門部付近の腫瘍に多くみられ(p=0.0004)、gastric combined typeとpyloric gland typeには組織学的に高分化型例を多く認めた(p=0.0209)。gastric foveolar typeはリンパ節転移を高率に認め(p<0.001)、そのki67値はpyloric gland typeよりも高かった(p=0.01)。生存率を比較するとgastric foveolar typeは最も予後不良であり、pyloric gland typeは他の群より予後良好であった。【結論】肝内胆管癌における胃型粘液発現は腫瘍の進展、予後と深く関わり、gastric foveolar typeは悪性度が高い腫瘍といえる。

Significance of intrahepatic cholangiocarcinoma and gastric-type mucus manifestations. Report on the occurrence of cancer and abnormal mucus in hepatobiliary bladder cancer. The gastric mucus MUC5AC and MUC6 are of particular concern for the development of intrahepatic cholangiocarcinoma tumors and the development of tumors after treatment. 100 cases of intrahepatic cholangiocarcinoma were surgically resected, and clinical pathological analysis of MUC5AC (45M1, CLH2), MUC6 (CLH5), and MUC2 (Ccp58) was performed using immunohistochemical staining (positive cases were determined with more than 10% of positive cancer cells). Of the two types of antibodies, MUC5AC has 40% positive results, MUC6 has 20% positive antibodies, and MUC2 has 9% positive antibodies. MUC5AC and MUC6 staining results: MUC5AC(-)/MUC6(-);null type(n=43)、MUC5AC(+)/MUC6(-);gastric foveolar type(n=36)、MUC5AC(-)/MUC6(+);pyloric gland type(n=11), MUC5AC(+)/MUC6(+); gastric combined type(n=10)の4 groupにcategorizationされ, MUC2 positiveの9casesは, gastric foveolar typeとgastric combined typeにのみ recognitionめられた. gastric foveolar type and gastric combined type, gastric foveolar type and gastric combined type, and gastric combined type and pyloric gland type, and histologically well-differentiated type, multiple and high-grade tumors (p=0.0209). Gastric foveolar type はリンパパ転movable を高にcognitionめ(p<0.001), そのki67値はpyloric gland type よりも高かった(p=0.01). The survival rate is relatively good for the gastric foveolar type and the best for the pyloric gland type. [Conclusion] Intrahepatic cholangiocarcinoma, gastric type mucinous lesions are present and the progression of the swelling is low, and the gastric foveolar type is high in severity and the swelling is low after treatment.
