被験者は、京都府立医科大学附属病院歯科を受診した早期発症型歯周炎患者10名、成人性歯周炎患者20名、およびボランティアとして本研究に参加した歯周炎をもたない大学職員20名、計50名(男性19名、女性31名、平均年齢43.7±1.6歳)とした。歯肉溝滲出液(GCF)はHouらの方法を改良して、歯周ポケットあるいは歯肉溝に挿入したペリオペーパーから採取し、その容量をペリオトロン8000により測定した。引き続きプロービング深さ(PD)を測定し、早期発症型歯周炎患者、成人性歯周炎患者のPD4mm以上の部位よりの試料を歯周炎罹患群(それぞれEOP群、AP群)、歯周炎をもたない大学職員のPD3mm以下の部位よりの試料を健常群(Control群)とした。同時にプロービング時の出血(BOP)、歯肉炎指数(GI)についても検査、記録した。GCF中のカルプロテクチン(CPT)レベルは市販キットを用いて行った。GCF中のCPT濃度を比較したところ、EOP群で2.49±0.20μg/μlであり、AP群(1.85±0.19)、Control群(0.77±0.09),とそれぞれp<0.05、p<0.0001と有意な差を認めた。CPT濃度とPDの関係を調べてみたところ、EOP群でr=0.52、p<0.01であり、PDとの間に相関関係が認められ、これはAP群とPDとの相関関係(r=0.38、p<0.05)よりも強いものであった。CPT濃度とGIの関係を調べてみたところ、EOP群でr=0.57、p<0.001であり、GIとの間に相関関係が認められ、これはAP群とGIとの相関関係(r=0.53、p<0.01)よりも強いものであった。CPT濃度とBOPの関係を調べてみたところ、EOP群においてBOP+(n=24)2.75±0.22、BOP-(n=6)1.43±0.24μg/μlで両者の間には有意な差(p<0.01)が認められた。これはAP群(BOP+(n=24)2.02±0.21、BOP-(n=6)1.17±0.35)において認められた有意差(p<0.05)よりも大きなものであった。以上の結果より、GCF中のCPTは成人性歯周炎と比較して早期発症型歯周炎においてその濃度が高く、また臨床的指標との相関関係も強いことから、成人性歯周炎以上に早期発症型歯周炎においてCPTが有用な歯周疾患診断指標となる可能性が示唆された。

The subjects included 10 patients with early onset periodontitis and 20 patients with adult periodontitis at the Department of Dentistry, Kyoto Prefectural Medical University Hospital, and 50 university staff (19 males and 31 females, average age 43.7±1.6 years). The method of oral exudate is improved, and the oral exudate is measured. The PD of patients with early onset periodontitis and adult periodontitis was measured. The PD of patients with early onset periodontitis was measured. At the same time, bleeding (BOP) and periodontitis index (GI) during surgery are checked and recorded. The GCF's "CPT" is a very useful tool for marketing. CPT concentrations in GCF were 2.49±0.20μg/μl for EOP group, 1.85±0.19 for AP group, and 0.77±0.09 for Control group. The correlation between CPT concentration and PD was adjusted. The correlation between CPT concentration and GI was adjusted, and the correlation between EOP group and GI was adjusted (r= 0.57, p<0.001). CPT concentration and BOP concentration were significantly different (p<0.01) between BOP+(n=24)2.75±0.22 and BOP-(n=6)1.43±0.24μg/μl. The AP group (BOP+(n=24)2.02±0.21, BOP-(n=6)1.17±0.35) has significant difference (p<0.05). The above results indicate that CPT in GCF is more likely to be useful in the diagnosis of periodontitis than in the early stage of symptomatic periodontitis, and the correlation between CPT and clinical indicators is strong.
