目的:口腔がん術後患者に適用した口腔内装置が、梧下時の舌運動動態に及ぼす影響について超音波画像診断装置を用いて評価し、口腔内装置のリハビリテーションへの有効性を明らかにすることを目的として本研究を行った。対象と方法:対象は当病院を受診した口腔がん術後患者のうち、下顎歯肉がん術後の7名を下顎義歯群、舌がん術後の7名を舌接触補助床(PAP)群とした。また対照として、全身状態および顎口腔系に特記すべき異常および欠損がない健常成人22名を、健康成人群として検討した。方法は、口腔内装置の未装着時、装着時について、超音波画像診断装置(東芝メディカル社製Nemio17、SSA-550A、東京)により描出された梧下時舌運動の画像から、(1)舌中央部の陥凹深度、(2)陥凹時間、(3)口蓋接触時間、(4)総舌運動時間の測定を行い、さらに反復唾液嚥下テストにより嚥下機能の客観的評価を行った。その結果、以下の知見を得た。1.下顎義歯群において、装着時の陥凹深度は未装着時と比較して有意に増加した(p<0.05)。また、装着時においては、陥凹深度が有意に健常成人群より延長していた(p<0.05)。2.PAP群において装着時の口蓋接触時間、総舌運動時間は未装着時と比較して有意に短縮した(p<0.05)。また陥凹時間は未装着時、装着時とも健康成人群と比較して有意に延長した(p<0.05)。3.反復唾液嚥下テストでは、嚥下回数および積算時間において、下顎義歯群、PAP群に有意差は認められなかったことより、日腔内装置の装着は、口腔がん術後患者の梧下時舌運動の援助に有効であることが示された。また、超音波画像診断装置は、舌機能への日腔内装置の効果を明らかにするために有用であり、リハビリテーションへの応用も期待できるものと考えられた。

Objective: To evaluate the application of ultrasonic imaging diagnostic devices in oral cavity for patients after oral surgery, and to clarify the effectiveness of oral cavity devices. Methods: 7 patients after oral surgery, 7 patients after oral surgery, and 7 patients after oral surgery. 22 healthy adults and 22 healthy adults were investigated for abnormal and deficient oral health conditions. The depth of depression was significantly prolonged (p<0.05). 2. The time of flap contact and tongue movement of PAP group with and without PAT were shortened intentionally (p<0.05). The time of depression was significantly longer in healthy adults than in unloaded adults (p<0.05). 3. Repeated saliva ingestion, number of swallowing cycles, cumulative time, mandibular prosthesis, PAP, installation of intra-oral devices, oral surgery, assistance in lower tongue movement The ultrasonic imaging diagnostic device has the function of detecting the effects of the ultrasonic imaging diagnostic device on the patient's body.
