To develop a new four-dimensional image-guided radiotherapy (IGRT) system, the following systems were developed.1. Four-dimensional treatment planning systemFour-dimensionally reconstructed CT images were obtained using MD-CT and gating signals. With these images, the movement of lung tumor could be more precisely evaluated.2. Respiratory gating systemThe relationship between the abdominal wall movement of a patient and lung tumor movement was evaluated. As a result, the correlation was so high (0.97). Therefore, the abdominal wall movement can be used as a surrogate signal for lung tumor.3. Clinical application of SBRTTwo-hundreds and thirty patients have been treated with SBRT technique. Ninety percent of high local control rates had been achieved for the patients with T1NOMO and T2NOMO tumor. Serious complications greater than CTCAE Grade 3 were 1.5%. The clinical results for the patients with pulmonary oligometastases were also promising.4. Development of an IGRT systemA new four-dimensional IGRT system was equipped with gimbaled X-ray system and ultralight C-band linear accelerator on a rigid 0-ring structure. The accuracy of the static beam positioning was 0.1mm.
