Development of a four-dimensional planning system and gated irradiation system for stereotactic body radiation therapy.

为建立完整的四维立体定向全身放射治疗系统，开展了以下研究工作：1.呼吸门控系统为了扩大立体定向放射治疗的范围，对于呼吸功能较差的患者，门控系统是理想的选择。在本研究中，建立了一个流程和一个压力门控信号发生器和一个辅导系统。2.用于SBRT的图像配准：CT、MRI和FDG(Flt)-PET之间的图像配准。该系统既可用于靶点勾画，又可用于疗效评价。3.SBRT的临床应用：已有180例患者使用该技术进行治疗。分别评价原发肺癌和继发性肺癌的临床疗效。4.JCOG0403SBRT治疗I期肺癌的多机构II期研究正在进行中。

To establish an integrated four-dimensional irradiation system for SBRT (stereotactic body radiation therapy), the following research had been performed.1.Respiratory gating systemTo expand the stereotactic radiotherapy for a patient with poor respiratory function, a gating system is ideal. In this study, a flow and a pressure gated signal generators and a coaching system was established.2.Image registration for SBRTThe image registration between CT, MRI and FDG (FLT)-PET was performed. The system could be used for both target delineation and evaluation of response.3.Clinical application of SBRTAs many as 180 patients have been treated with this technique. The clinical results of primary lung cancer and secondary lung cancer was evaluated respectively.4.JCOG0403A mutli-institutional phase II study of SBRT for stage I lung cancer is ongoing.
