ヒト生前脳室周囲白質軟化症(PVL)症例における胎児好中球の機能、炎症性サイトカイン濃度およびこれら相互の関連について観察することによって出生前PVLの病態形成における胎児の炎症反応の関与を明らかにすることを目的とし研究を行った。九州大学病院周産母子センターにおいて分娩に至った早産症例の中で、頭部エコー検査で出生前PVLと診断された症例を対象とし、子宮内感染(絨毛膜羊膜炎)。胎児ジストレン・新生児仮死など、PVLおよびCPと関連性を認める周産期合併症との関連性を検討した。対照には、分娩週数ならびに出生体重が合致した早産症例を用いた。分逸直後の臍帯血から調整した好中球ならびに血漿を用い、好中球の機能は走化能、O_2-産生能、脱顆粒を測定した。ついで、好中球を活性化する炎症性サイトカイン(インターロイキン(IL)-1β,IL-6,IL-8,G-CSF,tumor necrosis factor-α(TNF-α))および好中球を抑制する抗炎症性サイトカイン(IL-4,IL-10,IL-11,IL-13,sTNFγ,IL-1ra)の臍帯血中濃度を測定した。対照群との比較から、出生前PVL症例では、好中球O_2^-産生能、および血漿IL-1β、TNF-αが有意に高値である、IL-6、IL-8、G-CSFに有意な差はなかった。この結果から好中球のO_2^-、および血漿IL-1β、TNF-αの出生前PVLの発症への関与の可能性か示唆された。

The study was conducted to investigate the relationship between fetal inflammatory response and pathogenesis of prenatal PVL in cases of prenatal PVL. Kyushu University Hospital: Maternal and Child Birth, Head Examination, Diagnosis of Prenatal PVL, Intrauterine Infection (Chorioamnionitis) The relationship between fetal mortality, neonatal mortality, PVL and CP is discussed. The number of weeks of delivery and the number of preterm births are all related. After the separation, the function of the plasma, the chemical energy, the O_2-generation energy and the degranulation energy were determined. The concentrations of inflammatory cytokines (IL-1 β, IL-6,IL-8,G-CSF,tumor necrosis factor-α(TNF-α)) and anti-inflammatory cytokines (IL-4,IL-10,IL-11,IL-13,sTNFγ,IL-1ra) in umbilical blood were measured. In contrast, Prenatal PVL cases showed higher levels of IL-1β, TNF-α, IL-6, IL-8 and G-CSF than those in control group. The results showed that O_2-, TNF-α and IL-1β in plasma were closely related to the development of prenatal PVL.
