项目摘要

本年度の前半は,主に合意が相互行為的に達成されていく現場として,救命センターでの治療場面に焦点をあて,患者の死が確定的となる場面の分析を行った.特に医療者の間で患者の状態が「死」として確定していく会話上でのやり取りを示し,医療者間で患者の死が物語り形式の語りによって家族と共有されているプロセスを明らかにした.また後半は,不確実な状況下における医療者と患者家族の相互行為の構造を明らかにし,現代日本社会における医療場面の意思決定過程に関する実証的な研究を行った.本研究で明らかになった会話上の構造は,突然の死が自然な死として認識可能となる一つのステップを示唆している.説明から意思決定過程というプロセスは,家族が治療中断という決断をしやすいようなデザインになっていた.また説明自体が,説明可能性(accountability)を高める形で組まれていることで,その後の提案の正当性Gegitimacy)が高まるという仕組みになっていた.さらに家族における交渉(negotiation)にも着目し,妊婦とその家族との相互行為を分析し,その中でどういったnegotiationが行われ,家族の再編に際して合意がどのように成立していっているのかを分析した.妊婦の身体性がリソースとして使われることで,新たな家族を迎えるプロセスが調整され,その中で,胎児をどのような参与者として扱うのかというnegotiationがなされていた.
