项目摘要

医師不足、医師または診療科の偏在により地域医療を担う人材の確保が課題となっており、将来の医療の担い手のcareer preferenceの動向が注目されている。初期臨床研修医の将来の勤務地域の希望について、へき地・町村部における従事意思の実態と、研修期間中の変化を明らかにすることを目的とした調査を実施した。対象と方法：臨床研修プログラム検索サイトを利用して全国の臨床研修指定病院臨床研修担当へ郵送にて調査を依頼、同意の得られた294施設において、所属する2年目の初期臨床研修医を対象に、各施設の担当者を通して2017年11月に無記名のWeb調査の協力を依頼した。調査項目は、将来（卒業10年後）の大都市圏、地方中核都市、町村部、へき地（長期、短期：1年以内）における従事意思、診療科希望であり、初期研修開始時・修了時について4段階リッカートスケールで回答を依頼した。結果：551人から回答を得た（有効回答率14.2％）。・研修開始時および修了時の、将来の町村部・へき地（長期）・へき地（短期）の従事意思は、それぞれ開始時45.4%、24.5%、51.4%、修了時50.8%、24.0％、65.0%であった。・研修開始時と修了時の従事意思の変化については、町村部とへき地（短期）の勤務について、研修開始時で希望なしだった研修医のうちそれぞれ約2割・4割弱が修了前で希望ありに変わっていた。・研修開始時から修了までの間で、36％が希望する診療科を変更し、その理由は、診療科への興味、診療科への適性、研修した時の印象の順に多かった。考察：約半数の研修医が将来町村部における従事を前向きに考えており、へき地（短期）であればおよそ6割が希望していた。町村部とへき地（短期）の勤務について、開始から修了前にかけて、約2割・4割弱が希望ありに変わっており地域医療研修の経験が影響を与えた可能性が示唆された。
