项目摘要

本研究の目的は,遺伝カウンセリングにおいて臨床心理士が提供できるほどよい「心理的援助」の要件を探索することである。これまで我々は(1)実際の遺伝カウンセリングでクライエントが示す非言語的コミュニケーションのうち,心理的配慮が必要な徴候を抽出しつつ,(2)遺伝医学を修得した医学部生が想像したクライエントの心情および必要だと考えられる心理的配慮の内容を検討し,職種の違いを超えて必要とされる普遍的な心理的援助のあり方について検討してきた。その結果,遺伝病には「暗いイメージ」が先行し,漠然とした不安が喚起されやすく,ある病が遺伝との関係の中で捉えられるとき,「自分が特別な存在に変化してしまう」という,自分自身の存在意義をも含めた恐怖が生じやすいことが示された。しかし実際のカウンセリング場面では,そのような不安が言語化されることはまずなく,クライエントの心の揺らぎは,頑なな意思表明,脈絡のない多弁,何度も繰り返される陳述,問いかけに対する意外な応答などを通して推察する必要があった。また,面接前の待合室でのクライエントの態度や,面接後に見送る際に緊張がほぐれたクライエントから漏れたコメントから,重要な情報が得られたことが多く,このようなごくわずかな接点から援助の糸口を見出すことが重要であることが示された。なお医師の立場からは遺伝医学的情報を正確に伝えることの難しさ,先入観や偏見を解きほぐすことの難しさ,クライエントの心理的状態の見極めの難しさなどが実際に心理的援助を行うときの制約として挙げられた。このような困難を克服するために他職種との広い連携が必要であるという認識は浸透していることが確認されたが,情報漏洩や意見の不統一が起こらないよう緊密なチームワークが必要であり,そのあり方については今後も実践を通して検討し続ける予定である。
