本研究の目的は，臨床心理学の観点から医療コミュニケーション教育を検討し，新たな方法論を提示することである。本年度はまず予備的研究として，これまで収集してきた歯科医師および歯学部生に対する自由記述式質問紙調査のデータを質的に分析し，歯科医療関係者が「心」や「治療者－患者関係」に関して有する素朴な概念の構造を検討した。その結果，歯科医師が患者の「心」に対して何らかの手当てが必要だと感じた状況の多くが，治療者－患者関係の中で体験される難しさ（「コミュニケーションに対する不全感」「治療に対する認知のズレへのコントロール不能感」）と，患者のパーソナリティ理解に基づく対応の難しさに関連していることが示された。また歯科医師が「心への手当て」として実行していることは，「説明する」「傾聴する」「患者の意向を尊重する」「苦痛を与えないよう配慮する」などであった。学生から得られた「心」に関する自由連想記述をKJ法を用いて分類したところ，「奥底にある豊かな意味」「精神病理の原因追求への手がかり」「今，ここでの体験への気づき」「他者の心の不可侵性」の４群に大別され，学生によって優勢なイメージが異なっていることが示された。以上の検討を通して，医療コミュニケーション教育の実施にあたって重視すべき観点は，①「心」が喚起する多様なイメージを尊重しつつ，②異なるイメージをもつ学生同士の対話を通して，③他者および自己とのコミュニケーションを生き生きと体験できる機会を提供することであると考えられた。

The purpose of this study is to explore the new methodological implications of clinical psychology from medical to educational perspectives. This year's study will examine the conceptual framework of the "heart" and "therapist-patient relationship" between dentists and dental students. As a result, dentists have difficulties in understanding the patient's "heart" and in understanding the condition. The dentist said,"The patient's heart is in the right place,""The patient's intention is respected,""The patient's pain is in the right place," and "The patient's intention is respected." The four groups of students who have obtained the "heart" and the "inviolability of others" are classified into four groups: "the bottom of the mystery","the pursuit of psychopathology","the present of the body", and "the superiority of the student". The above discussion is open to the public, and the implementation of medical education is valued. ① The "heart" is aroused by the multi-media and respect; ② The different media is communicated with the students; ③ The opportunity for others to have a chance to live in the body is provided.
