今年度は,本研究の予備調査として実施した「精神疾患患者の主介護者の語りのパターンと患者に対する感情との関連」について学会発表を行った。主介護者の語りのパターンを対応分析を用い分類したところ,4パターンに分類できた。地域重視パターンは地域で周囲の支援を受けながらやる方針だが,他家族からの情緒的支援は少なく,肯定的感情は中程度だった。自己責任パターンは初回入院患者の主介護者が多く,介護を自分の問題として受け止め，自力で介護をしていく方針で肯定的感情は最も高かった。前向きパターンは再入院患者の主介護者が多く，他家族から情緒的な支援を受け，回復の希望を持っており肯定的感情は中程度だった。悲観パターンは患者との関係に困難を感じ，他家族からの情緒的支援も少なく，懸念や苦悩にとらわれ，肯定的感情は最も低かった。よってパターンに応じた支援の重要性が示唆された。また,精神科病棟に初回入院のうつ病患者が、退院後の生活で抱く困難点を明らかにし,精神科外来における再発予防に向けた支援を検討することを目的で昨年度実施したインタビュー調査の結果をＭ－ＧＴＡを用い分析した。研究協力者は,入院中から退院後にかけ,波長が合った援助を受け,気軽に行ける場を獲得するなど周囲からの【支えを自覚】していた。退院後は,やれることだけを見極め行動し,他者とうまく距離を取り,自ら気持ちを発するなど入院中に習得した【対処行動を活用】していた。そのような中,過去の自分が出来ていたことを取り戻すことで安堵したり、過去の自分と比べ出来ないことで自信を失うなど【過去との対比】を行うことで自身の状態を評価し,揺らいでいた。また,医療者に遠慮し,表出しても分かってもらえないという不満足感から【支援の不十分さ】を感じる者もいた。そして、調子の悪い波をキャッチし,不透明な先行きを懸念するなど【不安】を増強させていた。
