本研究の目的は、わが子がNICUに入院した直後で心理的に危機状態にある親が、子どものいのちに直接関わる重大な治療に関する意思決定を行う場面において、どのような看護ケアが行われているかを明らかにすることであった。本年度は、研究協力者として、臨床経験およびNICUでの看護経験が5年以上の看護師3名から6つの場面についてインタビューによるデータが得られ、質的に内容を分析した。その結果、以下の点が明らかとなった。1.意思決定を促進させる支援として、親と子どもの関係性が深まるような看護(面会するための環境を整える、子どもの様子を詳しく知らせる、子どもへのタッチをすすめる・見守る)が行こなわれていた。2.医師が行う医学的な情報の提供内容について、その理解を確認し、意思決定のための情報が不足していないかどうか査定していた。3.親が1つの選択肢を選ぼうとするときの不安や恐怖、また選択した後も、その決定を認め支えたり、親が決定した治療方針を受け入れる際の心理的な葛藤に対する看護を行っていた。4.夫婦間、また他の家族成員(祖父母)において、意思決定にどのような影響力を持っているか確認し、相互間の影響を調整していた。5.看護師が、意思決定への支援が困難であると感じたのは、意思決定の開始において親と医療者の治療方針が異なる場合であった。
