平成20年度の研究進捗状況思春期を生きるI型糖尿病患児へのインタビュー調査の実施と分析本年度は、慢性外来に定期受診している患児9名からの協力が得られた。性別は男子4名、女子5名であり、年齢は11歳〜18歳であった。調査期間は、平成20年11月〜平成21年3月である。インタビュー内容は、療養行動に関しては、食事、運動、自己血糖測定、インスリン自己注射、生活習慣、の5つに焦点をあてて、これらの療養行動を行う際に、家族・友人が患児とどのように関わってきたのか、小学生の頃から現在に至るまでにどう変化したのか、またそれらの過程において、抱いた感情や患児の行動をインタビューした。分析は、承諾を得て録音したインタビューデータからを逐語録を作成し、研究協力者が語った内容を分析した。分析結果からは、性別で比較した場合、男子と比較して女子のほうが将来の進路や就職・恋愛などに関しての不安を訴える傾向にあった。また友人関係においては男子は公表せずに徹底した自己管理の方法を選択しているが女子はそのようにする者と、信頼できる友人に病気について告知し協力を得ている者がいた。女子は同じ糖尿病を生きる仲間とメール交換や外来受診日を同じ日にするなどして交流を持ち相談しあっていた。発達段階で考慮した場合、中学生・高校生は自己管理を自立して行い学校生活において特別な事象がない限りは他者の協力・援助を受けないで生活していた。今後、再度インタビュー協力者ヘインタビューの内容の信憑性について確認するとともに、信頼性、真実性の確保のために小児家族看護学分野、社会学分野の研究者より指導・助言を受け、分析を進めていく予定である。さらに、インタビュー協力者を増やし、これまでのインタビューで明らかにされていることが他の患児においても説明可能であるのか、相違点や新たな知見がないのかに関して継続比較分析を行っていく予定である。面接調査における倫理面に関する問い合わせや相談は現段階では生じておらず引き続き倫理的配慮に留意していく予定である。
