本研究では、社会不安障害(SAD)の記憶ネットワーク(記憶構造)の変容過程の提案と認知療法プログラムの開発を行い、その妥当性と有用性を検証することを目的としている。本年度の主な成果は以下の3点である。(1)記憶構造の変容過程の妥当性を検証するため、前年度に引き続き大学生を対象とした実験的研究を実施した。介入によって顕在的連合は変容したが潜在的連合は変容せず、各連合の変容に明確な関連が示されなかった。また、介入終了3ヵ月後の不安症状(生理的反応)が介入終了時の潜在的連合の強度と関連する傾向が認められた。したがって、潜在的連合は顕在的連合に遅れて変容することに加え、介入後の症状を予測すると考えられ、潜在的連合を評価することで再発リスクが低減する可能性が示唆された。(2)認知療法プログラムの提案と有効性を検証する目的で、(1)の参加者に対して認知療法プログラムとエクスポージャー法を実施し、効果を比較した。その結果、認知療法プログラムはエクスポージャー法よりも、否定的評価に対する認知を修正することが示された。したがって、本研究で提案した認知療法プログラムは、記憶構造と矛盾する情報の入手を促進することで介入効果を高めると考えられ、SADの治療として有効である可能性が示された。(3)SAD患者に対する認知療法プログラムの適用のための予備的知見を得るため、大学生を対象とした実験的研究を実施し、SADの記憶構造の中核である他者評価の認知に関わる脳部位を検討した。その結果、否定的な評価が与えられると扁桃体や後帯状回等が賦活する一方、肯定的な評価が与えられると線条体が活動する傾向が示された。これらの脳部位は記憶構造と関連する可能性が推測され、認知療法プログラムの適用可能性や記憶構造によるアセスメントの妥当性を保証する上で有効な指標となる可能性が示唆された。

The purpose of this study is to investigate the development, appropriateness, and usefulness of cognitive therapy for the development of memory structures for social disturbance (SAD). The main achievements of this year are as follows: (1)A study on the adequacy of memory structure and its transformation process was conducted in the past year. (3) Cognitive therapy for patients with SAD is applicable to the study of cognitive assessment of other subjects. The result shows that negative evaluation and tonsillar posterior gyrus are active, positive evaluation and tonsillar posterior gyrus are active. The likelihood of association between memory structures and cognitive therapy, the likelihood of applicability of memory structures, and the likelihood of indicators of appropriateness of memory structures are also discussed.
