Development of a patient-family-professional collaboration model to support discharge for psychiatric long-term inpatients

通过本研究建立的放电支持模型由三个要素组成。与患者的合作包括对患者心灵和希望的确认和尊重、出院支持声明、共同解决问题、探索新的可能性等。第二，与家庭合作包括构建家庭护士关系、确认和尊重家人的心和希望、评估家庭能力和可能的护理等。最后，跨专业工作包括完成IPW的核心要素、减少出院障碍的要素、为IPW创造氛围和文化的要素等。

The discharge support model developed through this study was consisted of three elements. The first point, cooperation with patient contained confirmation and respect for patient's mind and hope, declaration for discharge support, working on the problem together, exploring new possibility, etc. The second, cooperation with family contained constructing family-nurse relationship, confirmation and respect for family's mind and hope, assessment family ability and possible care, etc. The last point, interprofessional work contained core elements of accomplishment IPW, elements of reduce discharge barrier, elements of creating the climate and culture for IPW, etc. Through applying the model in clinical practice, it is hoped community care for long term inpatients will be promoted.
