项目摘要

本年度は、昨年度に引き続き、待機群と独立評定者を設定したパイロットランダム化比較試験を用いて、児童の不安症と抑うつ障害に対する診断横断的介入プログラムの有効性と作用機序の検討を行った。不安症や抑うつ障害を有する児童16名に対するパイロットランダム化比較試験の結果、有効性については複数の評価者 (臨床家、親、子ども) と多面的な領域 (診断、症状、全般困難、プロセス) における査定を用いて検討した。有効性の検討の結果、診断横断的介入プログラムの実施後において、主要効果指標である診断面接による診断の数の改善が示され、副次効果指標である自己評定と親評定の抑うつ症状の改善が示された。次に、作用機序については、子ども用回避行動尺度、子ども用快活動尺度、不安の馴化を測定するための自覚的障害尺度を用いて検討した。その結果、自覚的障害度は不安症状に対する疾患特異的な媒介変数となることが再現された。一方、回避行動や快活動は、不安症状や抑うつ症状に対する診断横断的または疾患特異的な媒介変数としては示されなかった。加えて、想定した作用機序ではないものの、快活動が診断に対する媒介変数となる可能性が示された。以上のように、本研究では、本邦初の児童の不安症と抑うつ障害に対する診断横断的介入に対するランダム化比較試験を実施した。児童青年の不安症や抑うつ障害に対する実証的研究が非常に少ない本邦の現状を踏まえると、本論文で実施した実証的研究は、研究的にも臨床的にも大きな意義がある。今後、本研究を足掛かりにして、不安症や抑うつ障害を有する児童青年やそれに伴う心理社会的問題を抱える児童青年にとって、有効性と有用性を兼ね備えた有益な支援が社会に普及し、実装されていくことが期待される。

This year, compared with last year, the study of the effective mechanism of intervention for diagnosis of cross-section of children's restlessness and impairment was carried out. 16 children with anxiety disorders were selected for the study.
