本研究においては、一元的な効用指標を用いた医療経済評価では療養者個人の多様な境遇を十分に捕捉できないという問題の解決のため、医療ケア面の視点と資源配分の適正化という経済政策面の視点を統合させた医療経済評価を構成し、医療資源を効率的かつ公正に配分するための理論の構築と実証方法の検討を進めた。まず、療養者個人の財・利用能力と厚生の評価指標に関する理論構築と実証手法の検討を行った。個人による看護サービス利用に焦点を当てて1財・2利用能力・2機能の下でのケイパビリティ・アプローチ定式化のモデル（後藤・小林 2018）を理論的に提起した上で、個人の生活像に関わる機能を特定して実証分析を行い、機能空間上に機会集合を導出できることを示した。次いで、個人の財・利用能力・達成可能な機能に基づいた医療資源配分における個人間差異の評価手法に関して実証分析を行った。入院患者および在宅療養者の機能達成のために投入されたサービスと利用能力の程度との関係を明らかにし、利用能力の特性を考慮すると最適とはいえない資源配分がなされていることを示した。以上から、(1)患者・療養者個人の社会サービス利用局面においては、患者・療養者の機会集合最適化を狙ってサービス資源配分が決定されていること、(2)それは資源空間における効用水準の最適化とは異なる可能性があること、(3)機会集合最適化の下で達成された機能は当人の利用能力の特性を考慮すると臨床的には効率的とはいえない可能性があることが示唆された。ただし、(2)、(3)に関しては現存のデータによる実証分析では限界があり、今後、時系列で追跡調査を実施し、達成された機能や機会集合の状況や財・サービス利用の変化を捉えて検証する必要がある。
