本研究の目的は初期アルツハイマー病に対する認知リハビリテーション（以下、認知リハ）は認知症の二次予防（進行予防）及び三次予防（ノーマライゼーションの具現化）に役立つのかを明らかにすることである。研究１では、認知リハの計画立案と効果評価に適したアセスメントツールの開発と検討を行う計画で、研究２では、個人実施型の介入の要素を組み入れた新しい形の集団実施型認知リハプログラムを開発し、その効果を検証する計画である。このうち、令和4年度は、介入計画立案および介入効果評価のためのアセスメントツールの作成と、認知リハプログラムの実施準備を行った。アセスメントツールの作成に関しては、介入計画立案用ツールとして、介入前に実施する包括的なテストバッテリーの検討を行った。評価対象として認知機能、生活機能、心理機能（例、自尊感情）を含めることを検討中である。介入効果評価用ツールとして、反復測定可能な評価指標について検討した。認知機能や生活機能等の軽度認知障害や認知症の基本的病態の評価に加えて、自尊感情や自己効力感といった参加者の心理機能や、認知リハ中の参加者の様子を評価する項目についても指標に加えることを検討した。しかし、個々の参加者の目標の達成度と個の変化をより的確に評価するには、いくかの課題もあり、現在、さらに検討を続けているところである。認知リハの実施準備に関しては、以前から申請者が定期的に行っている集団認知リハや、不定期に参加している認知症予防プログラムに参加しながら、本研究の認知リハプログラムの内容の再検討やマニュアル作成の準備を行った。また、海外で行われている初期認知症に対する認知リハプログラムに関する文献的検討も行った。マンツーマンの個別実施型の認知リハの要素を集団実施型の認知リハの中にどう組み入れるか、模索しているところである。
