项目摘要

がんゲノム医療において治療薬選択のためのコンパニオン診断は、結果が陽性の場合には治療薬適応の判断がなされるが、同時に遺伝性腫瘍診断になり、血縁者への影響が考えられる。陰性の場合は、遺伝性腫瘍の可能性は低下するが治療薬の適応にならない。コンパニオン診断を受けた患者の中には、血縁者へ情報提供をしない、あるいはできない場合もあり、血縁者の予防行動へ繋がらない。本研究は、遺伝性腫瘍診断あるいはコンパニオン診断を受けた患者・家族の認識・理解、診断結果の受けとめ、医療者への期待、家族に対する思いを明らかにし、患者・血縁者のがん予防行動、早期発見・早期治療、良好な家族関係形成に対する医療者間連携による効果的な対応を明らかにすることを目的とする。本年度には、鳥取大学医学部附属病院遺伝子診療科を遺伝性腫瘍診断およびコンパニオン診断を目的に受診したクライエントおよび家族を対象に、気分・感情の変化および遺伝カウンセリングに関する認識、理解度、満足度、検査への期待、家族への思い、心配・不安に思ったこと等について質問紙調査を実施するため、文献検討およびがんゲノム医療に関連した遺伝カウンセリングに参加し、クライエント・家族の思い、情報の理解、検査結果に対する思い、検査結果による家族への思い等について把握した。遺伝性腫瘍診断を実施したクライエントは、血縁者への影響を知る目的で受診していたが、家族に事前に伝えている場合があれば、検査結果によって伝える人もいた。コンパニオン診断を受けたクライエントは病状によって治療薬適応の期待は異なっていた。検査結果が陰性の場合、治療薬が適応されないことに失望する人、血縁者への影響が低下し安堵する人がいた。検査結果が陽性の場合、治療薬が適応され希望をもつ人、血縁者を心配する人がいたことを把握した。
